You agree to the terms and conditions.	The undersigned hereby seeks to participate in the Forest Path Work and understands that the Centre desires to ensure the safety of all participants. In order to assist Forest Path in this regard, I agree to complete the Confidential Medical history form provided. I am assured that the information provided will remain strictly confidential and will serve only as a guide in determining the appropriateness of my participation in the work and in meeting my needs before, during and after the ceremony. I have informed myself of the nature of the works, the needed preparations and the rules of the ceremonies. I commit myself to stay in the Centre until the end of the ceremony and to respect the directives given by the guardians named by the Facilitator of the session. By choosing to participate in the Forest Path works, I affirm that my engagement arises from a place of spiritual intent, and that I recognise these ceremonies as sacred expressions within the living tradition of the Forest Path. The plant sacraments we share are not offered as treatments for medical or psychological conditions, but as sacred allies in communion and transformation. Participation is voluntary and undertaken as an act of devotion—a step into the deeper current of one’s own spiritual unfolding. I understand that Forest Path services and gatherings may be recorded—serving as vessels for teaching, training, and the living record of our ministry’s unfolding mission. I understand that my participation in the events of Forest Path is voluntary and may involve physical, emotional, or spiritual activities. I agree to take full responsibility for my own well-being and actions during my time on the property. I hereby release and hold harmless the hosts, property owners, facilitators, and all affiliated persons from any and all liability, claims, or demands that may arise from participation in these events or from being on the premises, including but not limited to personal injury, emotional distress, or property damage. I confirm that I am physically and mentally fit to participate and that I will inform the hosts of any relevant conditions in advance.
